Introduction to Accident Investigation, OSHA 7505 Whoever is charged with managing your firm’s accident investigation should attend this course. By providing an introduction to basic accident investigation procedures and accident analysis techniques, the goal is to help you gain the skills necessary to conduct an effective accident investigation. The course is set up as a facilitated, interactive training session focusing on class discussion and group activities. Course topics to include: primary reasons for conducting an accident investigation employer responsibilities related to workplace accident investigations six-step accident investigation procedure Instructor: Ralph Oliveti, CSP (Safex) Date/Time: Thursday and Friday, May 27 and 28, 2010 Registration: 7:30 a.m. Program: 8 a.m. to 4 p.m. Location: Builders Exchange of Central Ohio, 1175 Dublin Rd., Columbus, Ohio Cost: $475 Credits Awarded: 1.4 OSHA CEU 2.34 ABIH (Industrial Hygiene) CM points 1 external meeting credit for safety council firms     Refund Policy Tuition minus 5% administration charge will be refunded if written cancellation is received two weeks prior to the program date.  Registration is nonrefundable within two weeks of the course date. Substitutions may be made at any time, but transfers to future courses cannot be made within the two-week nonrefundable period. The university reserves the right to cancel programs if necessary. In this event, the university's liability is limited to the full refund of your registration fee. Group Discount Any organization sending three or more students to a single program and same date is entitled to a 10% discount on each registration fee. Alumni Discount Alumni and currently enrolled full-time students of the University of Cincinnati are entitled to a 10 percent discount on their registration fees.  Degree and year of graduation must accompany all registrations requesting the discount.
